The following form is an attempt to better understand those who desire to volunteer. We do not expect perfect answers since we are not perfect people. The following questions are an attempt to appropriately place your talents as well as eliminate potential risks in our ministries. We expect complete honesty in your answers. Review and approval will be left to the leadership who have promised to uphold confidentiality.

I will be actively involved in FCC New Salem’s mission, values, and vision. I will do my best to uphold a Christian, Biblically-based character. I will communicate necessary information to my ministry leader. I fully understand the above statements and will fully comply.
